Investigator Registration Packet
All data fields on each of the following forms must be completed, signed and submitted simultaneously to finalize your investigator registration. In addition to these forms, a current CV must be included in your submission. Each of the following three forms must be completed and submitted with a copy of your CV to the Pharmaceutical Management Branch (PMB)
